Repeating light and dark bands across my inkjet output — what causes banding and how do I fix it?

Regular light or dark bands, or a repeating stripe pattern, appear across the output — most visible in solids, gradients, and mid-tones — rather than a single sharp white line. Most often caused by printhead-to-printhead / die-to-die stitching errors where overlapping head zones don't blend evenly, among other possible causes — a full step-by-step diagnostic and fix follows below.
