Thin white lines running in the print direction — how do I clear clogged or missing nozzles?

One or more consistent hairline white streaks run parallel to the paper/web travel direction, in a single color or all colors, and they stay in the same place sheet to sheet. Most often caused by dried ink or air in the nozzles after the press sat idle (decap/latency), so jets fail to fire on start-up, among other possible causes — a full step-by-step diagnostic and fix follows below.
