My foil or spot UV is peeling or won't stick — how do I fix the adhesion?

Hot or cold foil flakes, scratches off, or won't transfer cleanly, or the spot UV varnish beads up, fails to anchor, and peels — usually over heavy ink or coated areas. Most often caused by ink or coating incompatibility — inks with wax or anti-set-off/anti-slip additives, or silicone-containing uv coatings, create a low-energy surface the foil or varnish can't bond to, among other possible causes — a full step-by-step diagnostic and fix follows below.
