Faint repeated 'ghost' images are showing up in my solid areas - how do I stop ghosting?

A faint duplicate or shadow of part of the image appears within large solid areas, often lighter than the surrounding solid. It is most noticeable in darker colors and tends to worsen as press speed increases. Most often caused by anilox cannot refill and recover enough ink at speed for the ink demand (volume too low for the solid coverage on that plate), among other possible causes — a full step-by-step diagnostic and fix follows below.
