Why does my waste matrix keep breaking or labels lift with the matrix during inline die-cutting?

The waste-matrix skeleton snaps or ladders and stops the press, or die-cut labels pull off the liner and fly away with the matrix instead of staying on the web. Either failure forces frequent stops and slower running speeds. Most often caused by die cutting too deep and nicking the liner so the matrix grabs release fibers and breaks, among other possible causes — a full step-by-step diagnostic and fix follows below.
