A faint second copy of the image appears further down the page (offset/ghosting) — why?

A faint, repeated echo of text or graphics appears further down the same sheet, toner may transfer to the back of the following sheet, and heavy coats can look greasy or show blistering. Most often caused by hot offset: fuser too hot for the stock, so melted toner sticks to the fuser roller/belt and re-deposits one roller-circumference later, among other possible causes — a full step-by-step diagnostic and fix follows below.
