Colors are out of register or the back doesn't line up with the front — how do I fix inkjet misregistration?

Color separations don't line up (edge fringing/halos, blurry-looking text), or on duplex work the back-side image is shifted relative to the front. The shift may be constant or may drift and grow across the sheet or roll. Most often caused by web-tension variation or paper stretch/shrink from wetting and drying, especially on continuous-feed presses, among other possible causes — a full step-by-step diagnostic and fix follows below.
