Why is ink setting off — the back of my printed sheets is picking up ink from the sheet below in the delivery pile?

Wet ink transfers onto the back of the next sheet in the stack, leaving a faint mirror smudge of the image; it is worst on heavy solids and coated stock and can smear when the pile is handled. Most often caused by ink still wet — too heavy an ink film applied for the stock, among other possible causes — a full step-by-step diagnostic and fix follows below.
