Image is imaged in the wrong position / my plates won't register on press (punch and clamp alignment)

The image is imaged in the wrong place on the plate or comes out skewed/shifted, so plates don't register color-to-color on press or the image sits off relative to the punch/gripper edge. Most often caused by plate not seated against register pins/stops, or debris under the leading edge before punching/clamping, among other possible causes — a full step-by-step diagnostic and fix follows below.
