Toner scratches, flakes, or rubs off my coated or synthetic stock — how do I fix adhesion?

The image looks fine on the press but toner cracks, flakes, or wipes off when the sheet is folded, scratched, or rubbed, especially in heavy solids and on coated, laminated, or synthetic substrates. Most often caused by under-fusing: fuser temperature or dwell too low for the stock (media profile set for a lighter or plain paper), among other possible causes — a full step-by-step diagnostic and fix follows below.
