My folding cartons are cracking on the outside of the fold at the crease — how do I stop it?

The outer paper or liner splits or cracks along the crease when the carton is folded, especially on heavy or coated board and on folds that run across the grain. Most often caused by crease too shallow or weak — not enough bead formed, so the outer fibers over-stretch and split when folded, among other possible causes — a full step-by-step diagnostic and fix follows below.
