Vertical streaks or white lines run down the length of my prints — what's causing them?

Continuous lines run in the paper-travel direction: either dark or colored streaks, or white voids/scratches where toner is missing. On a color press the line is often present in only one color channel. Most often caused by a contaminated or failing charge device (corotron/scorotron wire or charge roller), where dirt produces streaks, among other possible causes — a full step-by-step diagnostic and fix follows below.
